IMMEDIATE ACTION STEPS
Your participation hinges on the complete submission of this checklist by Friday, April 3rd, 2026.
1. Zorts Sports App Registration (Mandatory Uploads)
- Step 1: Click the button below to accept the Hawks Mafia 14U Zorts Sports Invitation.
- Step 2: Create a Parent Account, then add your player (Name/DOB).
- Step 3: Upload a clear photo of the Birth Certificate.
- Step 4: Upload the Florida Sports Physical (EL2 Form).
- Step 5: Upload the Consent & Release From Liability (EL3 Form).
- Step 6: Upload a copy of the player's Health Insurance Card.
2. Required Documentation (Bring Paper Copies to Practice)
- Player/Parent Code of Conduct.
- Hawks Nation Consent & Release From Liability.
- Florida Sports Physical (EL2 Form) — Dated within the last 12 months. **If you do not have a valid EL2 Form dated within the last 12 months you will need to visit your primary care physician to update the form.
- Consent & Release From Liability (EL3 Form) — Dated within the last 12 months. **If you do not have a valid EL2 Form dated within the last 12 months you will need to visit your primary care physician to update the form.
TEAM COMMUNICATION
All official team messages, practice updates, and game schedules will be communicated through our dedicated GroupMe chat. We request that at least one parent or guardian per player joins the group to stay fully informed of any last-minute changes or announcements throughout the season.

PAYMENTS
Parents can submit all team payments through Venmo or cash.
